What to expect at your First Visit

FirstVisitBefore arriving for your first visit, please take a few minutes to fill out the Registration and History form located on this website under the "Patient Form" link on the left.  Since treatment for Obstructive Sleep Apnea is a MEDICAL service, please bring your medical insurance card.  If you have completed a sleep study and have the report, please bring it as well.

At your first visit, Dr. Burwell will review your medical history, complete a preliminary examination and explain how Oral Appliance Therapy might  help to treat your Obstructive Sleep Apnea condition.  We encourage you to ask questions and we will be glad to discuss any concerns you may have.

If you are a candidate for Oral Appliance Therapy for the treament of Obstructive Sleep Apnea, your insurance company will most likely require a precertification for the appliance.  We will collect the required documents from you and send a request to your insurance company.  This can take from one to four weeks depending on the company.  

Medicare does not require precertification, and in most instances, we can start the appliance treatment the same day as the consultation if all criteria are met for Medicare.

Open Communication

We believe that good care begins with open communication. We promise to speak candidly with you about your physician's diagnosis and recommendations and discuss any alternative treatments available.  We look forward to meeting you and providing the quality, comprehensive care you expect and deserve.

We will stay in communication with your referring physician and/or sleep physician.  Additionally, we will let your general dentist know that we are making you an oral device and, should the need arise, ensure him or her that we will refer you back to their office for any dental needs.  For the benefit of your health, we feel it is important to make sure everyone stays informed.
